Plan   ~ Record your wishes. Share your plan with: • those closest to you

Review  ~  Whenever there is a change in your health, personal life or living situation.


None of us knows what will happen to us in the future. Without warning any one of us could face a medical situation or be in a serious accident that would leave us unable to communicate our health care choices.
This is what Advance Care Planning is all about. It is a process of planning for a time when you cannot speak for yourself or make your own medical decisions.

The Desert Valley Hospice Society’s Advance Care Planning Team has a plan to explain Advance Care  Planning and its importance in our lives.  Information folders with handouts on both medical and legal aspects of Advance Care Planning are available.  A Team member can go through the material on an individual basis and explain the forms and completion process.

The Advance Care Planning Team  offers presentations to the public. The Desert Valley Hospice Society sponsors this community presentation and there is no charge. We are grateful for the generosity and the support from our donors and the encouragement that we have received in offering this program.
